Next Steps ACP is offered to individuals with serious illness who have potential for complications. The goals of this stage of planning are to assess the patient’s understanding of the disease progression (and related treatment options) and to identify goals of care. While it is assumed that full care and treatment will continue, the Next Steps ACP conversation helps to clarify when goals of care would shift based on the patient’s clarification of unacceptable outcomes.

Organizations interested in replicating the Next Steps ACP model will find it valuable to register representatives for all three days. Participants are welcome to register for any of the courses independently, based on their objectives.

Course Content:

• Day 1 (8/26): Next Steps ACP Design and Implementation ($250)
• Day 2 (8/31): Next Steps ACP Facilitator Certification ($475, or audit $250)
• Day 3 (9/1): Next Steps ACP Instructor Certification ($495)

Because ACP is not a one-size-fits-all conversation, ACP Facilitators must be trained to have conversations with individuals at different stages of health and in different settings of care. ACP Facilitators add value to an organization and community and are central to any program’s success.

The RC Instructor becomes certified to teach the RC ACP Facilitator Certification course. Replicating the RC approach to ACP is an important Instructor role. It requires ongoing education, competency assessment, and fidelity monitoring of RC Facilitators.

Organizations interested in replicating the NS ACP model will find it valuable to send representatives to all 3 days of our advance care planning certification courses; however, participants are welcome to attend NS ACP Facilitator Certification or Design and Implementation independently, based on their objectives.
