National First Steps® Conference

Respecting Choices® (RC) First Steps® (FS) Advance Care Planning (ACP) is the foundation for weaving ACP discussions into person-centered care and preventive healthcare for all adults, such as part of wellness visits. When integrated in the outpatient or community setting, and early in the course of healthcare, FS ACP discussions help to normalize the concept of planning and orient individuals to the importance of regular review and update of written plans.

Course Content:

Organizations interested in replicating the FS ACP model will find it valuable to send representatives to all 3 days; however, participants are welcome to attend FS ACP Facilitator Certification or Design and Implementation independently, based on their objectives.

National Last Steps® Conference

Advance care planning for individuals with serious, life-limiting illness is complex and challenging. RC Last Steps® (LS) ACP incorporates the nationally recognized Physician Orders for Life-Sustaining Treatment (POLST) Paradigm program into this stage of planning. Achieving desired outcomes of the POLST program requires a thoughtful approach to implementation strategies and person-centered ACP conversations. The RC LS ACP program’s systematic approach results in an effective and sustainable POLST program.

Course Content:

Organizations interested in replicating the LS ACP model will find it valuable to send representatives to all 3 days; however, participants are welcome to attend LS ACP Certification or Design and Implementation independently, based on their objectives.

National Share the Experience Conference

RC held the inaugural National Share the Experience (NSTE) in Milwaukee on September 7–10, 2016.

The purpose of the Summit was to provide a national forum for RC stakeholders to share their RC ACP implementation experiences and to gain a deeper understanding of the critical need for healthcare transformation to ensure person-centered care—care that honors an individual’s goals and values for future healthcare.

This conference provided individuals who had found solutions to improving person-centered care, using the RC model, a venue to share their learning with others.

There were so many great stories and the hope was that this event would provide opportunities to learn from a wide variety of groups and individuals who have implemented the RC model.

More than 200 people from 30 states and 3 countries attended the Inaugural NSTE Summit.

The Summit brought together ACP leaders and experts from state and community initiatives, ACP coalitions, and healthcare organizations to collaborate and exchange ideas and strategies focused on bridging the BIG gap between what people want based on their state of health versus what they receive in terms of advanced illness and end-of-life care.

Thanks to all who attended NSTE in 2016. Your participation helped make it a huge success!
