
First Steps® (FS) Advance Care Planning (ACP) is the foundation for weaving ACP discussions into person-centered care and preventive healthcare for all adults. When integrated in the outpatient or community setting, and early during the course of healthcare, FS ACP discussions help to normalize the concept of planning, and orient individuals to the importance of regular review and update of written plans.
Course Content
Day 1: First Steps® ACP Design and Implementation ($350)
First Steps® (FS) ACP Design and Implementation is intended for healthcare and community leaders (administrators, managers, program directors, and others) and intermediate to advanced level social work practitioners who are interested in implementing an effective First Steps ACP program within their organization or community.
Purpose
The purpose of this program is to describe the design elements in designing an effective FS ACP program within an organization or community.
Learning Outcomes
At the end of this course, the participant will be able to:
- Describe the Respecting Choices® evidence-based, systematic approach to creating a sustainable ACP program.
- Identify five leadership strategies that guide the cultural transformation of person-centered ACP.
- Explain four key elements and related strategies in designing a FS ACP implementation plan.
- Participate in small group problem-solving activities.
Day 2: First Steps® ACP Facilitator Certification ($420, or audit $200)
The First Steps® (FS) ACP Facilitator Certification course is designed to help participants learn the skills needed to facilitate foundational ACP discussions with any adult.
Target Audience
FS ACP Facilitator Certification is intended for healthcare providers (nurses, all levels of social workers, health educators, and physicians), clergy, and others involved in providing ACP facilitation.
Purpose
The purpose of this program is to assist participants in learning effective communication and conversation skills for person-centered ACP facilitation.
Pre-Course Requirements
- First Steps ACP Facilitator Online Core Curriculum; modules 1-4 (included in registration fee)
- Personal Engagement Activity
- Review of Advance Directive document appropriate for your organization/community.
Learning Outcomes
At the end of this course, the participant will be able to:
- Identify communication skills for Facilitating First Steps ACP conversations.
- Create strategies to transfer individual goals, values, and preferences to the written plan.
- Demonstrate beginning competency in facilitating First Steps ACP conversations through role-play activities.
- Become certified as a First Steps ACP Facilitator through final competency role-play demonstration.
Initial Certification Requirements
- Completion of the FS ACP Facilitator Certification Online Curriculum (Modules 1-4)
- Completion of the FS ACP Facilitator Certification classroom training (8-hour classroom instruction)
Ongoing Certification Requirements
Currently, RC has no ongoing recertification requirements. It is recommended that recertification requirements be established and monitored by the Facilitator’s sponsoring organization and RC Instructors. RC intends to establish recertification requirements in the future.
Day 3: First Steps® ACP Instructor Certification ($475)
The First Steps® (FS) ACP Instructor Certification course features strategies for teaching and evaluating the communication skills of others, and prepares participants to teach the FS ACP Facilitator Certification course.
Target Audience
FS ACP Instructor Certification is intended for healthcare educators, community leaders, intermediate to advanced level social work practitioners, and chaplains who are interested in becoming certified to replicate the FS ACP Facilitator Certification course in their own organization or community.
Purpose
The purpose of this program is to assist participants in becoming certified as Respecting Choices First Steps ACP Instructors.
Prerequisite
Participants must attend the First Steps ACP Design and Implementation course and be certified as First Steps ACP Facilitators.
Learning Outcomes
At the end of this course, the participant will be able to:
- Define Instructor responsibilities and common strategies for teaching the Respecting Choices First Steps ACP Facilitator Certification course.
- Facilitate a brief discussion on selected ACP topics.
- Apply three mentoring strategies to promote Facilitator certification and maintenance of competence.
- Apply at least three Instructor mentoring strategies during role-play activities.
Initial Certification Requirements
- FS ACP Facilitator Certification
- Completion of the FS ACP Design and Implementation course
- Completion of a FS Instructor Agreement
- Completion of the FS ACP Instructor Certification classroom training (8-hour classroom instruction)
Ongoing Certification Requirements
- Compliance with the FS Instructor Agreement
- Completion of curriculum updates and recertification requirements
Organizations interested in replicating the FS ACP model will find it valuable to send representatives to all 3 days; however, participants are welcome to attend FS ACP Facilitator Certification or Design and Implementation independently, based on their objectives.
