CTAC Innovations (Innovations) is a nonprofit affiliate of the Coalition to Transform Advanced Care (C-TAC) devoted to implementing new delivery systems for advanced illness and assuring that people receive care matching their preferences and values. Innovations develops long-term relationships with partners who share a commitment to comprehensive, evidence-based advanced illness care initiatives.
Currently, Innovations boasts 2 primary lines of service: Respecting Choices® and The Advanced Care Model.

The Advanced Care Model
In February 2017, C-TAC submitted the Advanced Care Model, its first Alternative Payment Model (APM) proposal, to the U.S. Department of Health and Human Services to increase patient access to value-based care.
The Innovations team works directly with health systems and other providers to implement the Advanced Care Model designed to improve quality, care experience, and cost outcomes for patients with advanced illness. This model includes comprehensive, person-centered care management; multidisciplinary team-based care; concurrent curative and palliative treatment; care coordination across all providers and settings; comprehensive advance care planning; shared decision making with patient, family, and providers; and 24/7 access to clinical support. The model also supports provider investment infrastructure, creates a return-on-investment (ROI) opportunity, and helps providers migrate from fee-for-service (FFS) to risk-based contracts.
