This series of six, 1-hour modules is designed for healthcare professionals who are interested in learning how to facailitate basic advance care planning (ACP) discussions, and want to enhance their skills when working with patients with progressive illness, and those who are likely to die within the next 12 months.
The content in this series is designed to teach the skills necessary to initiate ACP conversations, engage people in the ACP process, clarify information, explore individual goals and values, select a healthcare agent, and make referrals.The curriculum also focuses on learning the skills needed to help individuals complete written plans that represent their preferences and can be communicated to others. Additionally, the content helps the learner identify and address the multiple system challenges involved in implementing an effective ACP program.
- Module 1 Advance Care Planning and Advance Directives: Understanding the Language, Concepts, and Tools
- Module 2 Facilitating Basic Advance Care Planning Discussions
- Module 3 Creating an Advance Directive: Communicating the Plan
- Module 4 Facilitating Advance Care Planning Discussions (with adults with chronic, progressive illness)
- Module 5 Facilitating Advance Care Planning Discussions (with adults who are expected to die in the next 12 months, or those living in long term care facilities)
- Module 6 Making Advance Care Planning Work: Organizational Issues and Educational Strategies.
Completion of this course is a prerequisite to become certified as a Respecting Choices ACP facilitator.
