Nursing contact hours
Gundersen Lutheran Health System, Inc., is an approved provider of continuing nursing education by the Wisconsin Nurses Association, an accredited approver by the American Nurses Credentialing Center's Commission on Accreditation.
First Steps Facilitator Certification (Day 1) provides 7.0 contact hours. Design & Implementation (Day 2) provides 7.0 contact hours. Instructor Certification (Day 3) provides 7.0 contact hours. Online facilitator course provides 9.4 contact hours.
California
Provider (Gundersen Lutheran Bereavement and Advance Care Planning Services) approved by the California Board of Registered Nursing, provider #12245, for 7.0 contact hours for Day 1, 7.0 for Day 2, 7.0 for Day 3, and 9.4 contact hours for the online facilitator course.
Social Work Credits
Gundersen Lutheran, provider #1089, is approved as a provider for continuing education by the Association of Social Work Boards, www.aswb.org. ASWB approved period: November 15, 2009 - November 15, 2012. Gundersen Lutheran maintains responsibility for the program. Licensed social workers should contact their individual state jurisdiction to review current continuing education requirements for licensure renewal.
Social workers will receive 9.4 continuing education clock hours for the online course, 7.0 continuing education clock hours for Day 1, 7.0 continuing education clock hours for Day 2, and 7.0 continuing education clock hours for Day 3 for participating in this course.
California
Course meets the qualifications for 7.0 hours of continuing education credit for Day 1, 7.0 for Day 2, 7.0 for Day 3, and 9.4 for the online course for Licensed Clinical Social Workers as required by the California board of Behavioral Sciences, Gundersen Lutheran Bereavement and Advance Care Planning Services is an approved provider, #934.
Other disciplines will receive a certificate of attendance.
To receive continuing education credit, nurses and social workers must complete the online course and be present for the entire program for which they register. Completed evaluation is required for contact or clock hours.
Continuing education certificates will be given on site upon completion of the program and the program evaluation. Upon completion of the online course, a certification of completion will be available electronically.
