The content in this series includes skills at initiating conversations, engaging people in the ACP process, clarifying information, exploring individual goals and values, selecting a health care agent and making referrals. It teaches skills that assist individuals in creating written plans that represent their preferences and can be communicated to others.This online program consists of a series of six critical thinking modules designed for health care professionals who want to enhance their ACP facilitation skills in working with patients with progressive illnesses and those who we may not be surprised if they died in the next 12 months. Additionally, the content in this series assists the learner to identify the skills necessary to address the multiple systems challenges required to implement an effective ACP program.
NOTE: Completing this course is a requirement to become certified as a RC First Steps ACP, Next Steps ACP, and RC POLST Facilitator.
